At ICS, we believe you deserve care team that understands you. We help people with disabilities and chronic conditions live the life they want: healthier, stronger, and more independent.
For more than 25 years, ICS has worked alongside New Yorkers with disabilities to make sure they have what they need to thrive in their homes and communities.
What Makes ICS Different
ICS was the first Health Home in New York State created for people with physical disabilities. Our Care Managers and expert staff understand the challenges you face because they work to address them every day. When you join ICS, you get a team that helps you manage your health, find resources, and remove barriers that get in the way of living your life.
How We Support You
Your Care Manager can help you:
- Keep up with doctor visits and follow-ups
- Manage medications and treatment plans
- Access safe housing and healthy food
- Get the right wheelchair or mobility equipment
- Solve problems with homecare or benefits
- Prevent hospital stays and stay healthy at home
- Connect with peer mentors and community activities
Everything we do is built around our Three Pillars of Care: health, mobility, and independence. We recognize the importance of each area and the fact that your health, mobility and independence are interdependent. If you are not healthy, your independence suffers. If you do not have the right wheelchair, your health and your independence decrease. We support you in each area to help you live the live you want.
The ICS Experience
ICS care management is free for people with Medicaid. You keep your current doctors and services. We add the expert support that makes life easier. We advocate for you, coordinate your care, and connect you with resources so you can focus on what matters most to you.
