Partner with ICS to Deliver Disability-Competent Care
Health and independence are possible with the right support. ICS connects people with disabilities and chronic conditions to the care, resources, and community they deserve.
ICS has been New York State’s first Health Home created specifically for people with physical disabilities and complex health needs. Our Disability-Competent Care Management model helps participants stay healthier and on track while reducing preventable hospitalizations and improving quality of life.
Why Partner with ICS?
ICS extends the reach of your practice or organization by helping to address the non-medical factors that affect health. Our Care Coordinators work with your patients to:
- Coordinate doctor visits and follow-ups
- Access safe housing and food resources
- Manage medications and treatment plans
- Navigate Medicaid, transportation, and benefits
- Stay connected to community supports in their own language
Our Expertise
ICS’s expertise comes from decades of hands-on work with people with disabilities across New York City. Our staff includes specialists in mobility and rehabilitation, nursing, social work, and care management. We are known for our leadership in areas such as:
- Disability-competent primary care and provider training
- Wound prevention and treatment coordination
- Comprehensive mobility and equipment support
- Hospital transitions and recovery planning
- Homecare problem-solving and consumer-directed care
- Peer mentoring and independent living supports
- Addressing social determinants of health through benefits navigation
ICS is free for those with Medicaid.
How We Work with You
Our goal is to make collaboration easy and effective. When you refer someone to ICS, our team will:
- Follow up with the individual within 48–72 hours of referral.
- Confirm eligibility (Medicaid + qualifying conditions).
- Assign a dedicated Care Manager within days.
- Maintain communication with your team throughout the member’s journey.
