Who We Serve

ICS serves Medicaid recipients who:

• Live in NYC, Westchester, or the Hudson Valley
• Have physical disabilities and/or chronic health conditions
• Need help navigating complex health and social care systems

ICS services are free for eligible Medicaid members. Members can keep their doctors, home care aides, and health plans.

What We Do

ICS provides disability-competent care coordination that supports real life—not just medical visits.

Our care navigators help with:

• Scheduling and following up on appointments
• Navigating insurance and benefits
• Accessing home care, transportation, housing, and food supports
• Reducing avoidable ER visits and hospitalizations

We help people stay healthy, independent, and connected to their communities.
