Why ICS?
ICS was the first Health Home in New York State created specifically for people with physical disabilities and chronic conditions. For over 25 years, we’ve helped people live healthier, more independent lives.
At Independence Care System (ICS), we help New Yorkers with physical disabilities and chronic health conditions live healthier, more independent lives. Our Health Home care management connects members to medical care, housing, benefits, and community supports—while ensuring they have the mobility equipment and peer support needed to thrive.
ICS is free for those with Medicaid. Our care management does not replace your current services: it adds the extra support that makes life easier.
ICS partners with members and providers to deliver the coordinated care, equipment, and support people with disabilities deserve.
If you have Medicaid and live with chronic health conditions, disabilities, or social challenges, ICS may be right for you.
We can help if you:
- Have multiple health conditions
- Are struggling with housing, food, or transportation
- Need help keeping up with doctor visits or medications
You deserve care that works for you.
ICS is an extension of your care team. We help your patients or clients by managing the barriers that affect health: missed appointments, unstable housing, food insecurity, and lack of transportation.
We partner with:
- Clinics & Providers: Improving follow-up, compliance, and outcomes
- Home Care Agencies: Supporting clients after discharge and reducing re-hospitalizations
- Community-Based Organizations: Closing gaps in care and coordinating long-term support

We can provide staff presentations, community workshops, or one-on-one support to make referrals easy.
Making a referral is quick and secure:

Our team will:
- Follow up with the referred individual within 48–72 hours (is this the time it takes?)
- Confirm eligibility (Medicaid + qualifying conditions)
- Assign a dedicated Care Manager within days
There’s no cost to join—services are fully covered by Medicaid.
